• Tournament Roster Registration Form - Note:  If your principal will not verify student's school attendance, leave the Principal Signature Blank and have all parents complete and return the Parent Attendance Verification Form below.  Each student must have this form completed.  This form is ONLY for those teams that do not have principal support.
